Job Description:
A company is looking for a Case Manager RN (Remote).
Key Responsibilities
Assess, plan, implement, and coordinate case management activities
to evaluate members' medical needs
Develop proactive care plans to enhance members' short and
long-term health outcomes
Conduct assessments using various data sources to address complex
health and social needs
Required Qualifications
Active and unrestricted RN licensure in the state of Florida or
compact licensure in state of residence
3+ years of clinical experience
2+ years of case management, discharge planning, and/or home
healthcare coordination experience
Associate's Degree in Nursing (required); Bachelor's Degree
(preferred)
Certification as a Case Manager (preferred)
