Job Description:
A company is looking for a Care Coach II to support family
caregivers through empathy and technology.
Key Responsibilities
Engage and educate caregivers via phone and digital technology to
improve their wellbeing
Manage a caseload, assessing caregiver and care recipient needs
while creating care plan goals
Document coaching activities and collaborate with stakeholders to
enhance care plans and caregiver engagement
Required Qualifications
4+ years of experience in care planning or case management
Experience working with the elderly or adults with disabilities
Solid understanding of care planning and case management
principles
Proven experience accessing community resources
Ability to work 10-7 EST/9-6 CST, M-F
